A Clinical Study of Massive Upper Gastrointestinal Bleeding

Abstract

This survey has been conducted of 84 cases of massive upper gastrointestinal bleeding who were
admitted to the Department of General Surgery of Busan National University Hospital during 4
years from July to June 1973 and the following results were obtained.
The incidence of bleeding was 53 cases (63. 1%) in male and 31 cases(36.9%) in female. And
the greatest incidence of the disease occurred in the age group of 40^60 years.
The most common presenting symptom and physical "finding was hematemesis(79.8%) and next
melena (59.0%). .
Duodenal ulcer was the most common cause of bleeding(38.1%. The mortality was about 25.0% overall.
Exsanguination and sepsis were the leading cause of death.
An operation was required in 67 patients (35 emergent and 32 elective).
The mortality rate for emergent procedures was 35. 0% as contrasted with 8.0% for elective procedures.
The mortality for ulcer diseases requiring surgical treatment was 33.0% for emergent and 20.0% for emergent and 20. 0% for elective procedures.
As a result of this review the following recommendations are made; urgent diagnostic evaluation, rapid blood replacement, early operation in those who do not stop bleeding promptly, more elective surgical treatment of those who have bled significantly, and a controlled clinical study of the best definitive surgical procedure to use in the treatment of the patient with an actively bleeding ulcer.
